Legal basis:
Article 30 of the Act of 19 July 2019 on Providing Accessibility for Persons with Special Needs (Journal of Laws, 2022, item 2240)
Request for accessibility provision
Application details:
- provide your name, surname, address, phone, e-mail
- indicate what form of contact would best facilitate your architectural or information-communication related matter in the Office
- specify your preferred method of contact
- indicate how should we ensure accessibility to you
Methods of submitting the application:
- send an application on the address: Statistical Office in Opole, ul. Ks. Hugona Kołłątaja 5B 45-064 Opole
- leave the application in the Office's secretariat (ul. Ks. Hugona Kołłątaja 5B, 45-064 Opole)
- write an e-mail message on the address: SekretariatUSopl@stat.gov.pl
- send a fax on the number 77 423 01 25
- come ot the Office and meet Office's staff between 7 a.m. and 3 p.m.
Timeframes for processing the application:
- no later than 14 days from the date of submission of the application
- to two months in a difficult situation, of which you will be informed. We will set a new date
We provide you a support of other persons and technical support.
Where and when to submit a complaint?
Jeżeli nie zapewnimy Ci dostępności w wyznaczonym terminie przysługuje Ci prawo złożenia skargi na brak dostępności. Skargę możesz napisać do Prezesa Zarządu PFRON w terminie 30 dni.
If we fail to ensure you an accessibility in a set deadline, you have a right to lodge a complaint on lack of accessibility. Complaint can be submitted to the President of the Management Board of PFRON within 30 days.
